    3、我对在连续而激烈的比赛中可能发生的危险已有充分的认识，我保证本人的身体健康状况能够承受本次比赛，并具备近期健康检查证明书。

    4、我保证：本人在比赛中因受伤、遭遇意外事故或发生疾病时，对受到的急救处理方法及结果不提出异议。
